Formstack

Formstack is used for ad hoc forms that are needed such as the submit a claim form from the customer portal for each scheme and also feedback forms.

When logged in, go to the forms tab and either create a form and duplicate and edit an existing one. Amend the form as needed and then make sure you have set the correct settings for submissions.
